CPW Nominations

At the annual conference, CPW will present the President's Award, Circle of Achievement Award, and Spotlight Award to CPW members who have made an extraordinary contribution to the Collaborative Practice. Please list your nominations and a reason for your nomination below. Nominations may be anonymous, and you may submit this form as many times as you would like.   • Circle of Achievement Award

    This award goes to a someone who has gone over and beyond the call of their own practice and duty, using their own energy and other resources, to advance and better Collaborative Practice in Washington State.
  • Please provide any information you have as to why this person should be nominated for the Circle of Achievement Award.



  • Spotlight Award

    This award goes to a newer Collaborative Practitioner (5 years or less) who has made significant contributions to the collaborative community, and who demonstrates future leadership qualities.
